 If you don't see your insurance provider on the list, you may have other options.

Call our office at 614-587-7778 to determine if you qualify to be seen as a Self-Pay* patient.

*Due to Local, State, and Federal guidelines and recommendations, we do not accept Self-Pay patients who are active Medicaid, Managed Medicaid (CareSource, Molina, Buckeye Community Plan, United Healthcare Community Plan, Paramount, Anthem Blue Cross Blue Shield Medicaid/Ohio Blue Medicaid) and/or Medicare Beneficiaries.
